Under supervision and direction of the Director of Nursing Transitional Care (Registered Nurse (R.N.) or Nurse Practitioner), the Licensed Vocational Nurse (LVN) provides back-office support to Wellness Center physicians, performing nursing duties within the scope of the Nurse Practice Act for vocational nursing and may have responsibility for home care nursing, clinic nursing and the operations within the Tribal Wellness Center. The LVN may carry out duties in the care of patients within the home setting, Wellness Center or within the transitional care space. The LVN is not an independent practitioner and is mainly assigned to work in the Wellness Center. The LVN will exercise independent professional discretion and judgment in selecting and training of employees, caretakers, and home health providers.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Collect vitals, apply/change dressings, and care for wounds. Collect specimens for laboratory analysis. Assist physicians, R.N.’s in performing examinations, respiratory care and other treatments including minor surgical procedures and PAP smears.
  • Connect with hospitalized Tribal Citizens to assist in ensuring their movement into next level of care or re-entry to home with home health needs is done safely. Visit Tribal Citizens in hospital prior to discharge and at home to ensure receipt and proper usage of all ordered medications including after hours and on weekends.
  • Prevents readmissions through directly checking or connecting with those who are checking vitals and/or labs for Tribal Citizens with chronic conditions such as congestive heart failure, liver failure, kidney disease, and then reach out to the primary doctor who is caring for the Tribal Citizens to coordinate plan of care.
  • Provide nursing care to patients within the scope of the Nurse Practice Act for vocational nursing; prepare patients for physicians or registered nurses. Care for patients both in the clinic and at home during treatment; Providing ordered medical treatment in addition to administering medications using subcutaneous, intramuscular, intradermal, subdermal, and/or oral methods.
  • Assist physicians and R.N. in the administering, and performing of special treatments, examinations, and intravenous therapy. Accurately chart treatments and medications and maintain pertinent medical records. Implement patient care plans; participate in patient education, rehabilitation programs, physical, occupational, group therapy programs, and document data appropriately. Participate in quality improvement activities related to patient care. 
  • Observe patients' condition and behavior and report significant changes to the supervisor. Counsel, advise and educate patients regarding proper healthcare practices that will improve overall health. Set clinic or medical appointments and procedures; refer patients for specialty care to outside agencies utilizing knowledge of payor source to ensure appropriate referrals to outside agencies and programs. 
  • Conduct educational training or intervention activities for patients, family, and staff regarding recurrent health problems and health practices related to maintenance of wellness and prevention of diseases.  Oversees, educates, and provides quality assurance for contracted home health providers. 
  • Performs other duties as assigned to support the efficient operation of the department.

 EDUCATIONAL, EXPERIENCE AND QUALIFICATIONS

  • Minimum of four (4) years of experience as an LVN in an accredited Ambulatory care setting or home health agency with a minimum of 4 years of medication management is required. 
  • Or minimum of 8 years of Medical Assisting in a Ambulatory care setting is required. 
  • Minimum of two (2) or more years of experience within a home health and/or Hospice company with the ability to support and guide staff in the absence of the Director of Nursing and Transitional Care required.
  • Related, relevant, and/or direct experience may be considered in lieu of minimum educational requirements indicated above.

KNOWLEDGE, SKILLS AND ABILITIES (KSA)

  • Case Management and Transitional care Experience is preferred. 
  • Related, relevant, and/or direct experience may be considered in lieu of minimum educational requirements indicated above. 
  • Demonstrated knowledge of modern principles, techniques, and procedures of vocational nursing inclusive of vocational nursing processes and its application in patient care; licensure, accreditation, and regulatory requirements; principles and techniques of effective supervision and administration; principles of quality assurance and research; problem solving techniques, team building principles, and infection control. 
  • Demonstrated ability to analyze needs of nursing services and medical plan of care, equipment and supplies. 
  • Demonstrated ability to deliver effective educational presentations to patients, new mothers, and caregivers. 
  • Experience in identifying opportunities for improvement in plan of care consistent with contemporary patient care trends. 
  • Experience in establishing and maintaining systems of medical records and reports. 

REQUIRED LICENSES, CERTIFICATIONS AND REGISTRATIONS

  • At the discretion of the San Manuel Tribal Gaming Commission, you may be required to obtain and maintain a gaming license.
  • Current Vocational Nurse’s License issued by the California Board of Vocational Nurse and Psychiatric Technician Examiners required.  
  • Current Basic Life Support and Current IV Certification required. 
  • Select One of the Following:
    • ☒ Authorized Driver: Role requires operation or driving of Tribe-owned vehicles or driving patron vehicles. A valid driver’s license with an acceptable driving record as determined by the insurance carrier is required.
    • ☐ Driving Responsibilities: Role requires regular commuting between locations. A valid driver's license and vehicle insurance with minimum liability limits is required. Role will not operate or drive Tribe-owned vehicles or patron vehicles.
    • ☐ No Driving Responsibilities: Role does not require a driver’s license or insurance.
